Physician Credentialing Overview

Each year Mint Physician Staffing coordinates applications and renewals for many of the physicians working with our client facilities and practices. Emergency Medicine Credentialing
For Emergency Medicine job placements, the following items are usually required for physician credentialing:
- Copies of all Active State Medical Licenses
- DEA License registered in the State you will be practicing
- Board Certificate or letter of Eligibility, if applicable
- Copies of Medical Diploma and Post Graduate Training Certificates
- ECFMG Certificate, if applicable
- ATLS, ACLS and PALS, if applicable
- CME Certificates for the past two years, if applicable
- Case Logs and Procedure Logs for the past two years
- Prior Malpractice Insurance Certificates for the past ten years
- TB test within the past 12 months or chest x-ray report within the past 3 years
- Proof of Flu shot within the past 12 months
- Immunization records and/or titers
- Current I.D., Social Security Card and/or U.S. Passport
Hospital Medicine Credentialing
For Hospitalist job placements, the following items are usually required for physician credentialing:
- Copies of all Active State Medical Licenses
- DEA License registered in the State you will be practicing
- Board Certificate or letter of Eligibility, if applicable
- Copies of Medical Diploma and Post Graduate Training Certificates
- ECFMG Certificate, if applicable
- BLS or ACLS, if applicable
- CME Certificates for the past two years, if applicable
- Case Logs and Procedure Logs for the past two years
- Prior Malpractice Insurance Certificates for the past ten years
- TB test within the past 12 months or chest x-ray report within the past 3 years
- Proof of Flu shot within the past 12 months
- Immunization records and/or titers
- Current I.D., Social Security Card and/or U.S. Passport
Additional Credentialing Requirements
In addition to the checklists above, the following items are occasionally required for hospital-based job placements:
- Stroke or Trauma CME Certificates
- Drug Screen
- Mask Fit Test
- Physical Exam or Health Competency Statement from a Physician
- Fingerprinting
